Key Responsibilities
?? Conduct comprehensive in-home evaluations to assess patient needs and develop tailored care plans.
?? Collaborate with Physical Therapy Assistants (PTAs) to execute care plans effectively.
?? Facilitate improvements in patient mobility and independence through targeted rehabilitation strategies.
?? Maintain open communication with patients' physicians to ensure continuity of care and optimal outcomes.
?? Evaluate and document patient progress, providing updates to medical professionals as necessary.
?? Supervise a team of 2 PTAs, ensuring adherence to performance standards and best practices.

Required Skills
?? Current, unrestricted California Physical Therapist License.
?? Minimum of 1 year of experience supervising PTAs in a home health setting (preferred).
?? Valid BLS/CPR Certification and California Driver??s License.
?? Knowledge of local community resources and their relevance to patient care.
?? Strong understanding of individual development, human behavior, and cultural influences in healthcare.
?? Familiarity with state and federal homecare regulations (preferred).
?? Experience working with high-acuity patients and interdisciplinary teams (highly desired).
?? Proficiency in case management fundamentals (highly desired).
?? Exceptional organizational, interpersonal, and leadership skills.

Qualifications
?? Self-direction, empathy, and solid critical thinking capabilities are essential for this role.
?? A post-secondary degree in Physical Therapy or an equivalent qualification is required.
