Job Overview
We are currently seeking a committed and proactive Registered Nurse (RN) to become a valued member of our team in a hybrid capacity, combining remote work with essential in-person visits. Key Responsibilities
Manage patient care and attend necessary medical appointments.
Coordinate hospital visits, procure medical equipment, arrange home modifications, and facilitate additional supportive services.
Operate independently, maintaining a flexible schedule while traveling locally as required.
Deliver outstanding care and support to patients, ensuring optimal treatment outcomes.
Engage in travel for in-person visits, which will comprise 50%-75% of your time.
Required Skills
Active Oregon RN License.
Valid Oregon Driver's License along with Auto Insurance.
Proficiency in Microsoft Word and Excel.
Bilingual proficiency in Spanish is advantageous.
Strong ability to work autonomously while also functioning as part of a collaborative team.
Qualifications
A minimum of 1 year of Nursing Experience (preferred).
A minimum of 1 year of Case Management Experience (preferred).
Active RN License is required.
Openness to travel locally for in-person visits (50%-75%).
